You can make your own forms in Dart. To access forms, go to: Settings > Forms and create your own form from the top left of the screen:
You can create standardized forms for things like bug reports or feature requests.
Create/edit a form
A form has three main parts:
Name
Sharing (who can see the form: Hidden, Internal, Public)
Form fields (default, forms will always include title, dartboard, type, and status)
Note that: when creating a form, a dartboard field is required as that is where the form will go.
You can add properties and move around the order of the form fields to customize its final look:
Adding more than one assignee to a form
If you need to assign more than one person to a form, you will need to go to:
Settings > Properties > Assignees
From there, ensure you turn on the ability to add multiple users.
Preview your form
You can also preview what the form will look like to others from the More button (three dots) at the top of your form page:

Sharing forms
Dart forms can easily be shared with others as long as they have access (either internal, for team members associated with the project, or public, for customer and client access). To share a form with others, simply click on the link icon to copy the link and paste it into a Dart task, Slack message, or email:
When you add your form to a task in Dart, it will show as a link on the right-hand side of your task window and under your task description:
You can also copy the link from your task (to share it with others) or go to the form by clicking on the copy link and open link icons beside the form within the task itself.
Completed forms
When someone submits a form, the completed form will appear under the dartboard linked to the form.
Form notifications
To ensure you are notified when a new form is submitted, go to Settings > Notifications and adjust your notifications so that when new tasks (forms) are created, you get notified:
